Nurse Navigator – Full Time (Benefit Eligible)
Location: Abbott Northwestern Hospital, Radiology Department, 800 E 28th St, Minneapolis MN
Primary Objective:
Provides care management support to Oncology patients who require Interventional Radiology services integrated in support of their overall plan of care. This position will assess care plans, document, coordinate, monitor, and update the plan of care by collaborating with the assigned health care team.
Responsibilities:
- Conduct patient assessments to create the appropriate care plan to enhance patient outcomes.
- Initiate and implement necessary modifications to care plan in partnership with the provider team as necessary while ensuring goals are met to measure effectiveness of the plan.
- Collaborate with patient, family, and provider team to coordinate services and identify/resolve any barriers.
- Coordinate health care benefits with appropriate insurance providers. Identify alternate sources of funding if necessary.
- Lead and support transition and discharge planning that is appropriate for the patient’s care plan.
- Provide education for patients’ diagnosis, treatment, and quality of life.
- Coordinate, implement, and document plan of care and outcomes.
- Participates in quality and performance improvement activities.
- Other duties as assigned.
Qualifications:
- Bachelor’s degree in nursing required (RN).
- Two or more years of nursing experience in Oncology and/or Interventional Radiology preferred.
- Two years of clinical experience in outpatient and hospital settings is preferred.
- Licensed Registered Nurse – MN Board of Nursing required.
- BLS Tier 1 – Basic Life Support – Multisource required.
- Case Manager Certification preferred.
