IR Clinical Nurse – Full Time (Benefit Eligible)
Primary Objectives:
50% IR Oncology Coordinator: Provides care management support to a panel of Oncology patients who require Interventional Radiology services integrated and in support of their overall plan of care.
50% IR Clinical Care: Provides care management support to outpatients who require Interventional Radiology services.
Responsibilities:
- Attends weekly multidisciplinary Hepatobiliary (HB) conference and implements plans as necessary.
- Assigns cases for multidisciplinary Hepatobiliary conference, updates and maintains master IR HB list and database.
- Assists in the review of evening/overnight IR cases, consults, and add-ons.
- Post-procedure inpatient rounding, patient education, including discharge teaching.
- Discharge supplies and coordination of potential IR follow-up.
- Order entry for outpatients under IR guidelines and directives.
- Assists in clinical patient phone calls and IR triaging.
- Communicates with patient, family, and healthcare providers about changes in plan.
- Coordinates, implements, and documents plan of care and outcomes.
- Documents plan of care in electronic medical record.
- Participates in staff, departmental meetings and assists with the identification and resolution of problems, ideas, and opportunities.
- Participates in quality and performance improvement activities related to specialty care practice.
- Other duties as assigned.
Qualifications:
- Bachelor’s degree in nursing required (RN).
- Two to five years of nursing experience is preferred.
- Two years of progressive leadership through engagement in performance improvement and program development is preferred.
- Licensed Registered Nurse – MN Board of Nursing required.
- Licensed Registered Nurse – MN Board of Nursing required.
- BLS Tier 1 – Basic Life Support – Multisource required.
- Case Manager Certification is preferred.
